Instructions for filling the form:
1) Beneficiary : Name of the person undergoing the Anaemia Profile test.
Note: Payment to be made online at the time of Sample Collection at your home.
Step 2: You will receive an SMS or an Email from Thyrocare Labs. You need to confirm the SMS or Email in order to activate your lead.
Step 3: You would receive a call from Service Provider to check your convenient time and date for Home Sample Collection.
Step 4: Representative in your city will come to your home for Sample Collection.
Step 5: At the time of Sample collection at home, you need to pay online.
Step 6: An email with soft copy of the Anaemia test report will be sent to you, once your sample is tested. Hard copies of the report would be sent to your home directly in 2 to 3 days time.
